Metal and plastic implants

Prostheses made of metal and plastic are the most commonly used hip replacement implants. The ball and socket of the hip joint are replaced by a metal implant with a plastic spacer placed between them. The most commonly used metals include titanium and stainless steel.

 

This plastic is called polyethylene. The implant is fixed to the bone by one of two methods; it is either press-fit or held in place with glue. In the press-fit method, the implant is pressed against the bone and new bone is formed around the implant to hold it in place. When the implant is glued, a special bone cement is used to hold the prosthesis in place.

New implants are constantly being developed in an effort to make these implants last as long as possible. A recent development is the improvement of the service life of polyethylene used as a substitute. These so-called 'highly cross-linked' plastics are manufactured in such a way that they wear out more slowly than conventional plastics.

 

Metal-to-metal implants

Metal-to-metal implants use a similar material, but without the insertion of a plastic spacer between the implants. Metal-to-metal implants are becoming very popular as they have been found to have very good wear properties in the laboratory.

 

In addition, there have been some highly publicised recalls of metal-to-metal implants because they have been shown to require revision surgery (replacement implants) at a higher rate than standard hip replacement implants. As a result, metal-to-metal implants have changed from a very popular type of implant to one that is rarely used.

 

Ceramic implants

Ceramic hip replacement implants also use metal components that fit the bone, but the bearing surfaces (ball and socket) can be made of ceramic material. Ceramic hip implants are designed to be the most wear-resistant of all available hip replacement implants.

They are even less wear-resistant than metal-to-metal implants. Ceramic is more scratch-resistant and smoother than any other implant material. The older versions of ceramic implants had problems because they were prone to breakage, but the newer versions do not have these problems. For this reason, ceramic hip replacements are becoming a more popular implant.

 

Which is the best?

There is no clear-cut best implant. Although new implants are being developed to improve the design, sometimes problems are identified once a new implant is released. For this reason, some surgeons prefer implants with a good, long-term track record. Your surgeon should be able to clearly explain their reasons for recommending a particular implant for you.
